Send both completed pages with check to Kay Krebs.( Address is on Page 1 and make check payable to NSCM) The second page must be signed and dated. Also you can call the Nordic Hotline (414-299-9639) for the phone numbers of members who can send you an application and answer your questions or attend one of the monthly meetings.
